Job summary We are looking for a talented manager to join our integrated management team in West Norfolk. Main duties of the job Duties include: Manage staff within the West Norfolk Home First Hub and urgent community response (UCR)team, including recruitment, performance, professional development and appraisal. Lead the improvement of peoples' experiences of discharge from hospital and across the West Norfolk admission avoidance teams, through redesign of existing services to reduce and eliminate (as far as possible) delays and implement new service developments to promote and embed the home first ethos. This includes establishing and imbedding working with colleagues so that the HFH works effectively as part of the Integrated Urgent and Emergency Care hub for West Norfolk.
