We are looking for a motivated & enthusiastic Band 6 Physiotherapist to join the Multi-Speciality Optimisation Team at St. James's Hospital in Leeds. The successful applicant will join the integrated Occupational Therapy and Physiotherapy team, covering wards designated for those patients who no longer have medical care needs, yet require further therapy intervention prior to discharge or have complex social needs. The team works together to assess physical, cognitive and social needs to achieve and facilitate the best outcome for patients when leaving hospital.
The successful applicant will be able to demonstrate in their application form and at interview relevant experience including skills in acute assessment, autonomous decision making and discharge planning. Applicants should be forward thinking with excellent communication, team working skills and practice patient-centred care. The post includes supervision of others.

Main duties of the job
There will be opportunity for this team to be involved with a number of local & national collaboratives including discharge, frailty, dementia and deconditioning. We are looking for applicants with leadership qualities who will relish the opportunities to contribute, facilitate, design and lead service developments, innovations and audit within the team. Principal Duties & Areas of Responsibility
Clinical
1. To be professional and legally responsible and accountable for all aspects of your own work relating to the assessment and treatment of patients in your care undertaking clinical duties as an autonomous practitioner.
2. To undertake the comprehensive assessment and treatment of patients including those with diverse / complex / multi pathological presentation.
3. Using investigative and analytical skills to formulate individualised management and treatment plans including the development of comprehensive discharge plans.
4. To accept clinical responsibility for a designated caseload of patients and to organise this effectively and efficiently with regard to clinical priorities and use of time.
5. To demonstrate dexterity, co-ordination and palpatory skills in the assessment and manual treatment of patients.
6. To develop and deliver the physiotherapy element of the rehabilitation programme.
7. Use a range of verbal and non-verbal communication tools to communicate effectively with patients.
8. Assess patient understanding of treatment proposals, gain valid informed consent.
9. To ensure a high standard of clinical care for the patients under your management.
10. Formulate and deliver physiotherapy treatment programmes based on a sound knowledge of evidence based practice.
