NHS Continuing Healthcare (NHS CHC) is a package of care for adults aged 18 or over which is arranged and funded solely by the NHS. North West London Integrated Commissioning Board (ICB) is looking for individuals to join the service in a variety of roles and bands. The NWL team has come together under a new structure and this is an exciting opportunity to join a dynamic and positive team.
North West London Integrated Care Board is looking to appoint two full-time Learning Disabilities Case Managers. The post holder will report to the CHC Complex Clinical Lead and is required to have a sound track record of working with adults with Learning Disabilities and/or Autism, managing the LD caseload, and ensuring correct processes are followed in the delivery of the National Framework for NHS Continuing Healthcare & NHS Funded Nursing Care.
Main duties of the job
The post holder will:
1. Be responsible for leading on the key operational day-to-day areas of work relating to CHC for people.
2. Ensure that the CHC service is fully compliant with and follows the national frameworks.
3. Support joint working with the LA ICBs and clinical teams within North West London.
As an experienced nurse or AHP, you will have a working knowledge of the Mental Capacity Act, Safeguarding, experience of undertaking assessments, case and risk management, and the personalised care agenda, which will be essential for this role. You will complete all aspects of NHS Continuing Healthcare and strive to improve processes and services.
We are looking for innovative, forward-thinking, enthusiastic, and confident nurses or AHPs to be part of the skilled and dynamic team. Previous experience of working in NHS Continuing Healthcare would be an advantage, and a level of knowledge and experience of the framework is required.
The post-holder will undertake various functions relating to the National Framework for NHS Continuing Healthcare, which will include:
1. Undertaking health needs assessments.
2. Facilitating multi-disciplinary assessments.
3. Reviews and supporting individuals with case management.
4. Staff leadership and management.
The Post Holder will act as an advocate for individuals with learning disabilities, ensuring their needs and preferences are considered throughout the CHC process. This may involve supporting them in accessing appropriate healthcare services, coordinating with external service providers, and empowering them to make informed decisions about their care.
You will be an excellent communicator with sound interpersonal skills, able to manage emotive and sensitive issues with tact and diplomacy, work well under pressure, negotiate with confidence, and maintain a high level of productivity. You will become part of a diverse multi-skilled team that supports each other with their clinical knowledge to ensure the best personalised care for each individual client.

Job responsibilities
Operational
1. Conduct comprehensive assessments to evaluate the healthcare needs, functional abilities, and support requirements of individuals with learning disabilities.
2. Work collaboratively with the service user, their families, and a multidisciplinary team to develop a holistic care plan.
3. Coordinate and facilitate the implementation of the care plan, ensuring that services and supports are delivered in a coordinated and timely manner.
4. Work closely with a variety of stakeholders, including healthcare professionals, social workers, care providers, and specialist learning disabilities teams.
5. Identify, assess, and mitigate risks related to the care and support needs of individuals with learning disabilities.
Finance
Liaise with finance colleagues to ensure that placement costs are accurately reflected in the service budget.
Information Management
Document assessment findings, care plans, progress notes, and any changes in the individual's health or care needs.
Planning and Organisation
Regularly monitor the quality and effectiveness of CHC services provided to individuals with learning disabilities.
Policy and Service Development
Actively contribute to the continuous improvement of CHC services for individuals with learning disabilities.
Person Specification
Qualifications
* Registered RN5 Learning disabilities nurse, level 1 with current NMC registration.
* Educated to degree level.
* Evidence of continuous professional development.
Experience
* Knowledge of National Framework for NHS Continuing Healthcare and Nursing Funded Care.
* Sufficient years post-registration experience.
* Proven clinical leadership skills.
* Experience of establishing & maintaining good working relationships.
* Experience of working across multi-agency services.
* Experience of holistic assessment.
* Excellent negotiating skills.
* Ability to assess patients with complex needs.
* Ability to lead case conferences/MDT meetings.
* Ability to work independently and manage competing priorities.
* Able to communicate clearly and effectively both written & verbally.
* Able to deal with challenges and appeals with the support of the Clinical Lead.
* Understanding of the roles of different professional groups.
* Aware of Deprivation of Liberty Safeguards.
* Experience of completing Mental Capacity Act Assessments.
* Aware of financial management, especially in relation to brokerage and invoicing.

£51,883 to £58,544 a year includes high cost area supplements (HCAS), Outer London.
