Job overview We have an exciting opportunity for an experienced Community Psychiatric Nurse to join us at Britton House in Gillingham. This comes at a time of transformation in mental health services under the community mental health framework, and will give the successful candidate opportunities to work within mental health, together where there are new treatment pathways being set up with different interventions being offered to service users. This role will allow the clinician to focus on delivering these, but also develop skills in working with the wider population. The post holder will receive support from an established team and regular supervision. Main duties of the job Carry out and coordinate comprehensive, systematic nursing assessments which take account of relevant mental, physical, social, cultural, psychological, spiritual, and environmental factors, in partnership with service user and others through interaction, observation and measurement. Work collaboratively with other disciplines, service user and their carers to agree a holistic person-centred care plan that addresses the needs identified through assessment To deliver and evaluate safe, person centred care in partnership with the service user and their carers to support recovery. To act as a clinical leader supporting best practice in line with NICE guidance. To facilitate and review approved Quality Improvement initiatives within the care setting. The post holder must hold a full, valid, UK driving licence and have access to a car/vehicle to use for business purposes (unless you have a disability as defined by the Equality Act 2010). This will range from encouraging self-care to providing total physical care depending on the needs of the patient. Maintaining and promoting a safe environment taking account of infection control and identifying and reporting hazards and risks in a timely manner. Escorting and transporting patients as required as part of their care plan. Driving own vehicle as per needs of the service and as individual capabilities allow. Depending on the environment, serving drinks and promoting healthy choices as required to meet individual care plan needs. Working a full range of shifts as per the requirements of the service. May also be required, at times, to work at other Trust units and/or sites. Physically able to carry out all duties required of the care setting, following successful completion of physical interventions training as required. Ensuring a process and continuous improvement in the way services are provided. Person specification Training Qualifications & Registration Essential criteria RMN/RNLD, NMC registration, completed preceptorship and practice assessor training. Desirable criteria Significant experience at Band 5. Evidence of post qualifying continued learning.
