Nurse Coordinator – Single Point of Access

Single Point of Access are looking for an experienced nurse who is highly motivated and has excellent communication skills, to join our team based in Ellesmere Port.
Working as a Nurse Coordinator, the post holder will effectively manage referrals from a variety of care settings and utilise various care pathways, ensuring that patients are referred to the most appropriate service for treatment.
This is a non-patient facing role working within a call centre setting.
This vacancy is Fixed term: 12 months, Part time - 28 hours per week.
We offer a relocation package of up to £8,000.
The relocation package is for anyone who would need to relocate in order to take up employment with Cheshire and Wirral Partnership. To be eligible for the relocation payment, you must live more than 40 miles from the base/location of the post you are applying for and you would need to relocate to within 15 miles of the new base.

Main duties of the job
The post holder will be responsible for facilitating the management of patients with unplanned care needs by ensuring that patients are referred to the most appropriate service which meets their individual health needs, adopting a patient centred approach. This includes admission to hospital where appropriate, preventing unnecessary hospital admissions by utilising community services and facilitating a smooth transition of care between primary care, secondary care and community care settings.

Person Specification
Qualifications
* Registered General Nurse (RGN)
* Previous experience of facilitating clinical supervision
Knowledge and Expertise
* Extensive current clinical knowledge
* Evidence of ongoing professional development
* Excellent communication skills, verbal and written
Experience
* A Registered Nurse Adult, currently practising with post registration experience within a community/primary care setting
* IT literate with good keyboard skills
* Experience of triage / multidisciplinary / interagency working
