We are looking for an experienced Band 6 Occupational Therapist to join our well-established and integrated adult community rehabilitation service.

Our community rehab team assesses housebound adults within their home environment. By assessing people within their home environment, we are in a good position to significantly improve people's quality of life and promote independence at home by providing therapy as well as by jointly working with social services, physiotherapists, and other members of our integrated team, making this a very rewarding job.
Main Duties of the Job
Undertake complex, holistic person-centred assessments, treatment, support planning, and reviews of people with a range of health conditions in the community.
Carry a caseload and complete detailed assessment and treatment of patients within the borough of Redbridge.
Work as part of an MDT demonstrating excellent rehabilitation of patients with complex physical and cognitive needs.
Experience in equipment ordering and moving and handling risk assessment.
About Us
Starting with NELFT
NELFT places a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks, which will include mandatory training, systems training, and the allocation of equipment. As part of the process, new starters will have the opportunity to meet the executive team, senior managers, and attend a number of drop-in sessions focusing on engagement, health and wellbeing, and key processes. The induction will be held at our head office in Rainham, Essex.
Probationary Period
This post will be subject to a probationary period. Internal applicants are exempt from the probationary period (unless you are an internal applicant currently partway through a probationary period or currently a bank member of staff).
High-Cost Area Supplement
This post also attracts payment for High-Cost Area Supplement of 15% of the basic salary (minimum payment of £4,551 and a maximum payment of £5,735) pro-rata for part-time staff.

Job Responsibilities
Provide high-quality Occupational Therapy clinical expertise and service. Responsible for the delivery of a range of evidence-based interventions which promote independence, social inclusion, and normalization for service users. Ensure continuous responsibility for coordinating the care of service users allocated.
Order and review equipment to facilitate independence and reduce manual handling risks.
Identify barriers to rehabilitation - fatigue, cognition, psychological, complete assessment and treatment, signpost to appropriate services. Manage challenging clinical situations and plan interventions while ensuring regular evaluation of the service user's treatment plan. Critically evaluate the effectiveness of therapy interventions, in collaboration with service users, carers, and other professionals. Provide specialist clinical advice and contribute to clinical information on service users' progress by completing regular reviews within the duration of treatment. Work in partnership with a range of other services to provide support and treatment to service users to facilitate the safe management of their needs and conditions. Work within the safeguarding adults and children frameworks across the Trust.
Complete in-service training.
Supervise junior staff.
Take responsibility for an evidence-based clinical audit.

Qualifications
* BSc (Hons) Occupational Therapy degree or equivalent
* HCPC registered
Car Driver
* Car driver with a valid license
* UK license
Experience
* 3 years NHS/Community experience
* Experience in equipment provision
