Main duties of the job
You will work as part of an integrated team
You will work in partnership with patients and their families to develop and agree tailored health plans to address their needs
You will actively participate in service development activities
You will work in collaboration with other health care and service providers to ensure seamless delivery of services for clients and communities
You will deputise for the Team Lead when necessary
