Job summary A vacancy has arisen within the Triage Team for the West Place and to support unplanned nursing care and catheter clinics in the community. The applicant should ideally have excellent communication skills with a good understanding of IT systems. Previous Community Nursing experience would be beneficial for this position. Main duties of the job To triage incoming referrals assessing the patient's needs, determining the priority of care and processing the referrals accordingly. To contribute in the delivery of high standards of health care to patients within their own homes, care homes, the travelling community and in clinics, by assessing, planning, implementing and evaluating packages of care. The position will require visiting patients in their own environment to provide unplanned nursing care.
