The Role:
We are looking for a Transitions Coordinator to join our Supported Learning provision. This role will involve working closely with a range of stakeholders to support the transition of students into mainstream and SEND provision. This post will focus on Transitions of EHCP and High Needs Students including attending Annual Reviews and school liaison.
The role will aid in recruitment, support retention and ensure high quality progression routes for our supported learners.
Key Responsibilities:
1. Develop links with schools, feeder schools, special schools, local authorities to plan provision and transition for learners with difficulties and/or disabilities to ensure a smooth and successful transition with maximum inclusion.
2. Have a good understanding of the curriculum and support programmes in schools to enable learners to progress on to the relevant programmes of learning in college.
3. Work with the schools to identify learners who will progress on to college.
4. Work closely with the Supported Learning and SEND SCMs, Senior Support Workers, Senior Support for Behaviour and the Medical Nurse to ensure all relevant information is recorded and provided in a timely manner.
5. Provide continued support to the learners and college staff once the learner has progressed to college and settled on their programmes.
6. Develop strong, supportive and positive relationships with the parents/carers/learners to support the process of person-centred transition planning.
7. Provide initial information and advice about SEND and mainstream courses at New City College to parents/carers and prospective learners to inform them of course choice.
8. Attend annual reviews at schools and arrange observations of the learners who potentially will be attending the college.
Who You Are:
The successful applicant will have experience of working within education with High Needs and EHCP learners.
You will have excellent knowledge of the education sector, with the ability to build links with other education providers, feeder schools and NEET providers.
You will be familiar with the Annual Review process of an Education Health and Care plan and the outcomes for learners with SEND.
You will have excellent IT and presentation skills and an ability to forge strong working relationships with internal and external partners and stakeholders.
