Due to the retirement of a long-standing and much-valued nurse, we are looking for a motivated Practice Nurse to provide care for the presenting patient, treatment, chronic disease management, and evaluation of care.
Working in accordance with the NMC Code of Conduct, the post-holder will require experience in cervical cytology and baby immunisations, essential. The post will play a crucial role in planning and coordinating the work of the nursing team under the guidance of the Lead Nurse.
Main duties of the job
1. To develop, implement and embed health promotion and wellbeing programmes.
2. To implement and evaluate individual treatment plans for chronic disease patients that promote health and wellbeing.
3. To identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to patients' health.
4. To maintain accurate clinical records in conjunction with current legislation.
5. To ensure read codes are used effectively by all members of the nursing team.
6. To ensure the appropriate maintenance and use of chronic disease registers.
7. To implement vaccination programmes for adults and children.
8. To delegate clinical responsibilities appropriately (ensuring safe practice and the task is within the scope of practice of the individual).
9. To support the clinical team with all safeguarding matters, in accordance with local and national policies.
10. Provide care for minor injuries, dressings, and higher-level wound care, liaising directly, where appropriate with community and secondary care counterparts, escalating to a GP or referring onto community services e.g., leg ulcer clinic/tissue viability, etc.
11. Undertake opportunistic screening when clinically required e.g., blood pressure, BMI, smoking status, alcohol intake, etc.
12. Perform venepuncture and electrocardiography and suture and staple removal.
13. Assessing problems presented opportunistically by patients.

Person Specification
Experience
1. Of working in Primary Care.
2. Minimum of 3 years post-registration experience.
3. Knowledge of evidence-based practice and clinical governance.
4. Evidence of appropriate continuing professional development activity to maintain up-to-date knowledge and ongoing competence.
5. Evidence of working autonomously and as part of a team.
6. Proven ability to evaluate the safety and effectiveness of their own clinical practice.
Qualifications
1. RGN with current NMC pin (if from a nursing background).
2. Evidence of continuing professional development.
3. Current NMC registration.
4. Cervical cytology qualification.
