Are you passionate about working with service users in Later Life? If so, we have an interesting opportunity to work with our skilled, experienced and supportive multi-disciplinary team as a Band 5 CPN/Mental Health Practitioner. The team provides complex interventions for older people who have severe mental health problems, both functional and organic.
You will be working collaboratively with service users and carers to provide a range of service including evidence-based psycho-social interventions and medication management.
You will need excellent communication skills and be flexible and resourceful in your approach. You will need to demonstrate your ability and commitment to team working and a genuine interest in achieving wellbeing and positive health outcomes for older adults.
Main duties of the job
Main duties include:
1. To contribute to the planning, delivery and review of treatment and care.
2. To act as a key worker for identified service users for whom you are delivering defined, evidence-based therapeutic interventions.
3. To provide support to carers and enable them in their relationship with the service user.
4. In collaboration with service users and carers, to carry out safety assessments and develop safety plans.
5. To provide mentoring/training to students and unregistered colleagues.
6. To support practices which foster effective team work.
This post is open to newly qualified social workers. In line with AWP trust policy all newly qualified social workers (NQSW) will be recruited initially at Band 5 level, regardless of advertised role banding. All NQSW's are expected to successfully complete their ASYE within 2 years of employment, unless there are exceptional circumstances in which case it may be extended for a further 2 years.
If you have been appointed to an advertised Band 6 role this will be an automatic progression, following confirmation of your successful ASYE completion. If you have been appointed to a Band 5 post you will then be eligible to apply for a subsequent Band 6 vacancy upon successful ASYE completion.

Key local information:
You will be based at Kingswood Civic Centre which is the central hub for AWP South Gloucestershire Locality, with the majority of mental health teams in the locality sharing the same space. This allows for a positive supportive culture and enables joint working across teams.
You will be working across a large geographical and socially diverse area. The locality was chosen to be the pilot site for the implementation of 'Your Team Your Conversation Your Plan' which is replacing CPA. As part of the Transformation project, we have been at the forefront of implementing new ways of working with service users and carers.
We can offer you an induction programme, Clinical and Management Supervision, Appraisal, regular CPD sessions and ongoing training appropriate to your development needs. You will have the opportunity to engage in quality improvement initiatives and service development. You will have the opportunity to provide supervision to unregistered colleagues and to support the development of Student nurses and Occupational Therapy students on placement.
