Working Full Time: 37.5 hours per week
A lead specialist in a defined area of nursing, the post holder will be an identified key worker for a patient caseload, accepting referrals to agreed criteria, coordinating treatment and care, practicing autonomously as a core member of the Integrated Discharge team, with a specialist focus on Dementia patients.
The senior CNS possesses expertise in clinical risk evaluation, change management, and the provision of educational support to patients, carers, and colleagues across organisational and professional boundaries.
Main duties of the job
The post holder will be expected to demonstrate a higher level of practice in both direct care of patients and in planning and developing services to support patient care. Utilizing the skills of leadership, clinical expertise, evidence-based practice, and innovation, the post holder will facilitate and deliver care that is patient-centered, safe, and effective.
The post holder may be required to work on other wards/departments within the Trust at short notice in the event of hospital pressures/major civilian disaster.

The Trust is a busy district general hospital, and in 2022/2023, there were more than 503,342 patient attendances (inpatient, outpatient, and diagnostic), ranging from a simple outpatient appointment to major cancer surgery. This is an increase of nearly 10,000 compared to the previous year when there were 494,387 patient attendances.
