Detailed Job Description and Main Responsibilities
1. To demonstrate high-level communication skills to Service Users who have complex emotional, physical, and psychological conditions.
2. To effectively overcome barriers to communication.
3. To utilise highly developed interpersonal skills to facilitate effective communication in hostile or highly emotive situations.
4. To establish and maintain robust communication networks with clients, carers, team members, and external agencies.
5. To establish and maintain good working relationships with GPs and other partner agencies.
6. To represent the service at local and national forums as required.
7. To be aware and keep up to date with the body of knowledge related to practice in a specialist community mental health team setting.
8. To accept responsibility for delivering care to a specific caseload of patients.
9. To develop and exercise high levels of judgement and decision-making in the care of Service Users on the post-holder's caseload.
10. To work autonomously using sound decision-making skills within the clinical setting.
11. Assess the mental and physical health needs of designated Service Users in a community setting.
12. Lead on decision-making for her/his caseload within the MDT.
13. Formulate protocols for decision-making agreed by the MDT.
14. To undertake and record comprehensive risk assessments and devise risk assessment plans.
15. To work in a specialist advisory capacity regarding appropriate treatment plans.
16. To implement non-medical independent and supplementary prescribing as per NMC guidelines.
17. To provide an interface between nursing and medical colleagues to ensure patient needs are met.
18. To ensure service users' and carers' needs are meaningfully considered during care delivery.
19. To promote the rights and entitlements of service users.
20. To advise the MDT on the care and treatment of individuals.
21. To maintain clinical records as per trust policy.
22. To be highly visible within the team, providing both advisory and clinical leadership.
23. To develop the skills of clinical staff in assessment, formulation, and care planning.
24. To provide clinical leadership for the team.
25. To deliver Trust-wide training on mandatory and bespoke training sessions.
26. To participate in the Bronze on-call / Bronze on-site rota.

Essential Criteria
* Registered First level nurse
* MSc in Advanced Clinical Practice
* Nurse Prescribing qualification
Desirable Criteria
* Clinical based Masters degree
Experience

* Experience of working with clients with complex needs
* Experience of working at a senior clinical nurse level
* Compiling protocols and clinical guidance
