We have an exciting opportunity within Community Services for Support Workers, working within the West Kent locality to support individuals with a learning disability in accessing and participating in their local community.
You will work from various hubs within this locality and be expected to operate within community settings, supporting service users to plan and achieve outcome-focused sessions. Although your base will be in Gravesend or Dartford, you will need to travel across the locality as required.
Your role involves meeting the needs and aspirations of people with learning and/or physical disabilities through a person-centred approach, providing encouragement, support, and assistance. You will promote social inclusion, rights, and choices under supervision and within the framework of County and Directorate procedures, primary care legislation, and other relevant regulations, to enable individuals to live more independent and fulfilling lives.
Applicants should have proven experience in caring for and supporting individuals, which can include voluntary or paid experience in the public, private, or voluntary sectors, particularly with adults with learning disabilities. An awareness of the needs of individuals with challenging behaviour, autism, epilepsy, and other disabilities is essential.
You may be required to escort individuals on the service minibus to access community sessions.
Applicants should hold a Diploma Level 2 in Health and Social Care, NVQ Level 2 in Health and Social Care, or an equivalent qualification, or be willing to undertake this training. A satisfactory induction period, including completing the Care Certificate, medication workbook, and all mandatory training, is required.
