We are a dynamic, forwardthinking PCN team who are passionate about developing and delivering excellentquality local services to meet the needs of our patients.
Due to the changes in the ARRSScheme to introduce General Practitioners we are looking to expand our team byseeking to employ a newly qualified General Practitioner to work alongside ourpractices and PCN staff.
Our PCN staff currently include ClinicalPharmacists, Social Prescribers, Respiratory Physiotherapist, Care Coordinatorsand Health and Wellbeing Coaches.
Main duties of the job
We would like a newly qualified GP who can work independently and with other health care professionals to assess, diagnose and treat the conditions of patients attending within a primary care setting. To be professional, accountable, and responsible for all elements of clinical practice in accordance with the GMC code of conduct ensuring the safety of patients and quality of their care is primary.

These include improving the ability of practices torecruit and retain staff; to manage financial and estates pressures; to providea wider range of services to patients and to integrate with the wider healthand care system more easily.
BradfordCity 5 PCN is a large PCN with 10 practices and a patient list size of approximately56,000 patients. We cover the geographical area of Bradford Moor, Barkerend, Thornburyand Eccleshill. We have a significant workforce in place which brings wonderfulopportunities for in-house multi-disciplinary team working. We have awell-developed team which includes a Pharmacy Team, Social Prescribers, Healthand Wellbeing Coaches, Care Coordinators and a Respiratory Physiotherapist.
Followingthe release of ARRS GP funding we are looking for newly qualified (within 2years) to join our PCN on a fixed term salaried contact until 31 March 2025.

Job responsibilities
In general, the post-holder willbe expected to undertake the duties and responsibilities associated with a GPworking within primary care in a surgery on a PCN basis.
These include but are notexhaustive:
1. Using SystmOne to record data.
2. Triaging
3. Surgery Consultations,
4. Telephone Consultations,
5. Home Visits,
6. Prescription Queries
7. Long Term Care
8. Sick Notes
9. Dealing with queries, paperwork,reports, and correspondence in a timely fashion.
10. Developing care plans andpromoting healthy lifestyle strategies
11. Manage and process test results
12. Refer patients to relevantservices including secondary care
