Care Coordinator

£14.25 per hour (Nights)
I am looking for a Care Coordinator who is dedicated to working collaboratively to uphold the personal care of our service users, while respecting their dignity and independence at all times
In the role of Care Coordinator you will:
• Play a critical role in ensuring that residents receive high-quality, person-centred care
• Work closely with the care team to coordinate and manage care for residents, considering their individual needs, preferences, and goals
• You will be responsible for organising your shifts, administering and recording medication, liaising with health professionals and recording to the residents’ computerised care plans.
• You will assist with the induction, training and supervision of support staff.
• Continue professional development through attending to training, meetings and supervisions
For this interesting and rewarding role you will need:
• Experience of supervising a care team.
• You will have, or be working towards, NVQ3 in Health & Social Care.
• The ability to show empathy and understanding of the needs of the residents
• Excellent time management skills and ability to work well under pressure
• A good eye for detail, and are organised and methodical
• Ability to make balanced, clear and timely decisions
• Commitment to Health and Safety practices
