Preparing for Adult Life (PfAL) Service

We have two posts available. One post will involve having a caseload which may be countywide and the other will lead on our Duty Social Work activities. The work base will be discussed with the successful applicant and our staff work on a hybrid working basis.
The Preparing for Adult Life (PfAL) Service ensures young people moving into Adult Social Care are supported in a seamless, joined up way.
You will have the ability to work with young people with complex needs, as well as their families. You will be able to demonstrate an understanding of the legislative requirements in relation to the Care Act 2014, as well as other legislation including the Children and Families Act 2014. As part of this role you will be expected to carry out assessments and care planning under the Care Act 2014.
Due to the expectations and requirements of this role we are looking for applications from candidates who hold a professional qualification in Social Work.
