Job Summary: Spalding Primary Care Network is seeking an Advanced Clinical Practitioner to join their busy team. The post holder will be an integral part of the general practice team, working as and when required in each organisation and across Spalding PCN.
The post holder will have key responsibilities in delivering health services as outlined in the Network Contract DES Specification PCN Requirements and Entitlements.
Advanced Clinical Practitioners work independently within their scope of practice in primary care, using enhanced clinical assessment and treatment skills to assess and manage patients presenting with acute conditions, including minor illness or injury, abdominal pains, chest pains, and headaches. They will provide a rapid response to urgent patient requests for home visits and care home reviews.

Primary Key Responsibilities:
* Work as part of a multi-disciplinary team (MDT) within Spalding PCN.
* Assess and triage patients, providing definitive treatment or making necessary referrals.
* Advise patients on general healthcare and promote self-management.
* Perform specialist health checks and reviews within their scope of practice.
* Interpret ECGs and perform investigatory procedures as required.
* Support the delivery of anticipatory care plans.
* Provide an alternative model to urgent and same-day GP home visits.
* Communicate effectively with all colleagues across the MDT.
* Maintain accurate health records and documentation.
* Communicate complex and sensitive information effectively with patients and their families.
Person Specification – Advanced Paramedic Practitioner:
Qualifications:
* Educated to master’s degree level in a relevant area of expertise.
* Capabilities of advanced clinical practice as set out in the Multi-professional Framework for Advanced Clinical Practice in England.
Additional Skills:
* Able to manage undifferentiated conditions and identify red flags.
* Use complex decision-making to inform diagnosis and management.
* Actively take a personalized care approach.
* Provide clinical practice and CPD supervision.
Personal Qualities:
