The work is varied and challenging and will focus on pathway 2 of the D2A service. You will be supporting and assessing people within a rehab-focused bed-based setting, following an acute hospital stay. The core work will be undertaking statutory assessments within complex situations, so knowledge of the Mental Capacity Act 2005 and Continuing Health Care legislation is essential.
The successful applicant will be part of a multi-disciplinary team including nurses, physiotherapists, occupational therapists, discharge facilitators, and support staff within the Haywood hospital and private residential and nursing homes. The team also works closely with a number of external partners such as the CCG/ICSs and Staffordshire County Council. The work is on a short-term basis and cases are usually closed within 12 weeks; this means the successful individual will need to be able to work at pace and manage competing priorities.
On appointment, the post holder will be auto-enrolled in the SWLA and aligned to the appropriate British Association of Social Work Professional Capabilities Framework. The relevant SWLA 'stage' (i.e. stage 1 (ASYE), 2 (Social Worker) or 3 (Advanced Social Worker)) will also be agreed taking all prior Continuous Professional Development (CPD) and relevant experience into consideration. The post holder will be required to take responsibility for their own Continuous Professional Development.
