Job summary An exciting opportunity has arisen for a band 6 registered nurse to join our multidisciplinary team within the Wakefield Intermediate Care Unit, based at Pontefract Hospital. The Wakefield Intermediate Care Unit is a 22-bedded Unit that provides short term inpatient care to people who need extra support, care and rehabilitation following an acute hospital stay. Our expert multi-disciplinary team includes registered and non-registered nurses, physiotherapists, occupational therapists and a wider support team who work in partnership with our patients and their families to provide the care they need and to support their safe and timely discharge home. Main duties of the job Responsibilities include: Provision of high levels of clinical and professional leadership, supporting the management team to implement clinical and operational policies, protocols, and service / practice development and change. Demonstrating clinical expertise in the assessment, planning, delivery and evaluation of care to meet patient needs and Unit objectives. Coordinating and supervising shifts, ensuring effective communication and collaboration within the multidisciplinary team.
