We are looking for a permanent, full-time Occupational Therapist to join our Adult Services Short Term Support Service in Preston. This role involves working across Lancashire to support individuals in achieving their goals and living independently in their homes. Key Responsibilities Collaborate with social work colleagues, care providers, NHS, and other partners. Develop and coach a re-enabling approach and/or longer-term compensatory adaptations for people leaving hospital or avoiding admission. Conduct high-quality holistic face-to-face Occupational Therapy assessments. Set and review therapeutic goals and create person-centred, robust, proportionate support plans.
