Main duties of the job The successful candidate will work at Alma Medical Centre, Stockton providing social care to our patients with a focus on elderly and frail patients. This role will involve carrying out home visits for patient assessments, medication compliance and social interactions. Therefore, a current driving license is essential. The focus of this role is to provide frailty health checks, assessments, and support to our elderly population to improve their health and wellbeing. Providing the creation of personalised care and support planning is a series of facilitated conversations in which the person, or those who know them well, actively participates to explore the management of their health and well-being within the context of their whole life and family situation. Social prescribing link workers will connect people to community-based support, including activities and services that meet practical, social, and emotional needs that affect their health and wellbeing, such as day centres, charities, or community groups to reduce loneliness and improve patients' quality of life. Also connecting people to statutory services for example housing, financial and welfare advice. You will work closely with our GPs, Health Professionals, Practice Staff, and wider multi-agency teams to provide advice on lifestyle issues, assist and support patients in changing their unhealthy behaviours and refer appropriately.
