We are looking for motivated & enthusiastic District Nurse to work within our Integrated Neighbourhood Nursing Teams, supporting housebound/residential care patients across the 6 neighbourhoods in Barnsley.
The service is responsible for delivering nursing care, which includes wound management, chronic disease management, promotion of self-care, administration of intravenous antibiotics & terminal/palliative care.
Our District Nurses work as part of the team to lead & manage registered/non-registered staff, taking overall responsibility for the assessment, management & evaluation of care delivered to patients.

Main duties of the job
As District Nurse you will:
1. Have responsibility for performance & clinical activity in relation to patient safety, effectiveness of care interventions & patient experience.
2. Ensure compliance with clinical governance frameworks ensuring performance systems are in place & Infection Control policy/procedures are followed.
3. Be equipped with the knowledge, understanding, skills, attitudes & behaviours relevant to employment & act in accordance with the NMC Code Professional standards of practice.
4. Have high level communication/interpersonal skills to negotiate & establish collaborative partnerships with patients & care professionals.
5. Have a key role in training, education & staff development, actively participate in regular supervision & appraisals contributing to clinical leadership facilitating a culture of learning/reflective practice.
6. Have a flexible approach to working long days, weekends, bank holidays as the team provides a 365-day service.
7. Line management responsibility of staff.

Job responsibilities
JOB SUMMARY
The District Nurse will:
1. Adapt and provide a wide range of nursing care in home and community-based settings.
2. Be accountable for assessing, planning, implementing and evaluating care for patients and managing unpredictable situations flexibly and responsively ensuring this is of a high standard.
3. Be responsible for the management of generic caseload and clinic clients, coordinating care, whether anticipated or unscheduled, with individuals and their families, through acute illness, long term and multiple health challenges and at the end of life.
4. Lead and manage a team to deliver care in the home and community.
5. Prescribe according to assessed need from within the nurse prescribing formulary.
6. Participate with the training of learners.
7. Work in an integrated and partnership way with primary, secondary, social care, the independent and voluntary sector and others to improve the health and care of individuals, families and communities, particularly the most vulnerable.

* Registered General Nurse.
* District Nurse Qualification.
* Recorded Nurse Prescriber.
Experience
* Substantial post RGN registration.
