Job overview
We currently have a vacancy for a Community Learning Disabilities Nurse to work within the Sefton Borough, supporting adults who have a diagnosis of a learning disability with complex physical and mental health diagnoses. Work as part of a dynamic supportive MDT and participate in an on-call rota.
The post holder will manage their own caseload with the support of their supervisor and will have the opportunity to undertake further training within their role; preceptorship is available.
Main duties of the job
1. Supporting individuals with a diagnosis of a learning disability with mental and/or physical health issues.
2. Maintaining a caseload with support from your supervisor and team.
3. Supporting individuals with a learning disability in various aspects of their life, including acute physical health issues and through a variety of pathways both within the trust and externally.

Detailed job description and main responsibilities
1. Be professionally and legally responsible and accountable for all aspects of the post holder's own non-clinical activity.
2. Promote and ensure that all aspects of clinical care offered to people with learning disabilities are of a high clinical standard and evidence-based in line with professional codes of conduct and local policies and procedures.
3. Liaise with other members of the Learning Disability Team and other agencies, families, and carers to support the development of Health Action Plans for people with learning disabilities.
4. Take responsibility for the safe administration of medication to service users ensuring that NMC Regulations are adhered to.
5. Facilitate access to generic services and act as a link person for Primary Care Health Services.
6. Carry out specific Nursing interventions for people with complex needs under the supervision of Senior Nurses.
7. Regularly evaluate progress using appropriate outcome measures and change care programmes within guidelines set by a Senior Nurse.
8. Develop and monitor written information for patients and their families and/or carers.
9. Be aware of psychosocial issues, cultural and social differences, and consider the implications of these when formulating a care programme.
10. Develop risk management plans for clients on own caseload and with complex clients under the supervision of Senior Nurse.
11. Carry out intake assessments under supervision.
Person specification
Qualifications
* Registered nurse qualification – Learning Disabilities RNLD Current NMC Licence to Practice.
* Portfolio demonstrating evidence of learning and development.
* Training qualification ENB 998 or equivalent.
Skills
* Ability to communicate effectively at all levels with other professionals and with people who may have significant barriers to understanding.
* Demonstrates good standard of verbal and written communication skills including basic IT skills.
* Basic knowledge of national and local policy/good practice guidance relating to care of people with learning disabilities.
* Awareness of specialist assessment tools.
* Ability to work effectively within a multidisciplinary team.
* Able to plan and manage own workload.

* Must have the ability to travel within the area of work.
Knowledge/Experience
* Demonstrate experience at pre-registration level of working across a wide range of clinical areas.
* Experience of nursing care planning process for adults with Learning disabilities.
