Job summary Are you passionate about the benefits of stopping smoking and supporting people to improve their health and wellbeing? We have an exciting new role based in Primary Care Networks to proactively identify people who may benefit from stopping smoking and supporting them on their journey to becoming smoke free. This role will include identifying, contacting and supporting patients, keeping accurate records of all contact and making onward referrals where this is most appropriate. The role will also have an element of awareness raising both with staff and the wider community and will include travel accross Calderdale. Please note this role is part time at 30 hours per week and is fixed term until 31st March 2026. Main duties of the job Motivate, empower and support people to make an attempt to stop smoking. Use the primary care clinical systems to identify smokers from agreed target cohorts and proactively contact them to offer support in making a quit attempt. Provide brief advice on smoking and explain the range of support options that are available to enable people to make informed choices. Deliver high quality evidence-based stop smoking interventions in accordance with the NCSCT Standard Treatment Programme. (F2F & by telephone). To work with communities in order to increase understanding of stop smoking benefits and explore the values of delivering stop smoking services in community settings. Adhere to agreed protocols and procedures to issue stop smoking products to people. Follow up with patients in accordance with service protocols to enable outcomes (e.g. successful and unsuccessful quit attempts) to be recorded and to provide ongoing motivational and behaviour change advice and support. Respond effectively and in a timely manner to referrals made by other members of the practice and PCN team. Keep and maintain accurate patient records using both the General Practice clinical systems and Quit Manager software. (Support the Project Manager to) complete the required reporting on project activity for the project funders.
