Health and Wellbeing Officer - Adult Early Intervention Support Service Grade 6 (£34,416 - £34,416)

Croydon Council, Adults Social Care, is seeking to recruit a Health and Wellbeing Officer to work in the Adults Early Intervention Support Service (AEISS) leading the way in delivering vital first-point-of-contact services to adults in need of support.
Key Responsibilities:
1. Ensure timely and efficient responses to referrals and inquiries received.
2. Respond to phone calls and incoming referrals, ensuring that individuals receive the right care and advice promptly.
3. Maintain a customer-focused approach, focusing on early intervention, prevention, and supporting independence.
4. Ensure that individuals, carers, and families receive clear and accessible information and advice.
5. Work closely with individuals and communities to ensure that services are responsive and inclusive, particularly for vulnerable or hard-to-reach groups.
