CDDFT Community TB contact tracing Nursing Service is pleased to advertise a permanent Community TB Nurse Specialist post working within the Countywide service. The successful postholder will deliver all aspects of complex TB screening, contact tracing, diagnosis, management and treatment including neonatal vaccination, in out-patient clinics as well as in their own homes.
The successful candidate will utilise their outstanding organisation and communication skills to achieve best patient outcomes whilst being well supported by expert TB specialists to develop a range of competencies in delivering excellent TB care and support for this group of complex patients.
Main duties of the job
1. Undertake specialist, holistic, assessment, diagnosis, planning, implementation and evaluation of clinical care for a caseload of patients with potential or diagnosed TB disease.
2. Be knowledgeable in TB disease, pathophysiology, function and management.
3. Provide specialist advice to patients/carers.
4. Act as an accountable practitioner.
5. Maintain accurate records as an autonomous practitioner.
6. Act as point of contact for the wider local community teams.
7. Provide education, mentorship and leadership for junior colleagues and students.
8. Skilled at assessing and managing risk.
9. Accepts the responsibility for decisions made and actions taken.
10. No requirement to manage/monitor budget.

Job responsibilities
Management and Leadership

Clinical Responsibilities
1. NMC nursing registration.
2. To case manage patients with potential or a TB diagnosis within a community setting without direct supervision, reporting to the TB specialist nurse lead, as appropriate, utilising specialist history taking and examination skills.
3. Holistically assess patient need and develop a plan of care that is individualised and meets the needs of patients and carers through physical assessment skills.
4. Identify, manage and review adjustments in optimising TB therapies within agreed service protocols, through non-medical prescribing.
5. Notify UKHSA of all new active cases of TB.
6. Develop, implement and evaluate patient treatment plans and concordance through clinical checks, tablet counting, and providing directly observed therapy (DOT).
7. Communicate patient treatment plans.
8. Eligible BCG vaccination administration for neonates, children and adults.
9. Phlebotomy.
10. Review clinical results.
11. New entrant screening and contact tracing for all eligible patients.
12. Assist patients to identify and manage risk factors using behavioural change to self-manage their condition.
13. Provide specialist advice to patients/carers in relation to programmes of care which may be complex.
14. Provide sensitive information to patients, where there may be barriers.
15. Decision making/clinical judgment and problem solving.
16. Liase with multi-disciplinary teams, referring as appropriate.
17. To contribute to the development of service specific guidelines and protocols that are evidence based.
18. Work closely with key members of the team from primary and secondary care to alter the management plan as appropriate. Communicate and accurately document any changes in a timely manner.

Qualifications
Essential
* Professional knowledge acquired through degree or equivalent level of demonstrable professional knowledge/experience.
* Excellent communication and interpersonal, organisational and problem solving skills.
* Experience of managing a caseload of complex patients including clinical assessment.
* BCG vaccination administration including neonates or willingness to work towards.
* Physical assessment skills course or willingness to work towards on employment.
