Job Overview
A fantastic opportunity has arisen for a Social Worker to join a newly established Fairer Funding Team.
As a Social Worker, you will be an integral part of the Local Authority's new Fairer Funding Team. Your extensive expertise and knowledge of Continuing Health Care (CHC) and Children’s Continuing Care will assist in ensuring individuals are supported to achieve their personalised outcomes utilizing a strengths-based practice framework while also appropriately accessing alternative funding streams such as CHC.
Responsibilities
1. Conduct Social Care assessments for adults, carers, and children.
2. Complete reviews and any subsequent care and support planning.
3. Manage related adult safeguarding enquiries.
4. Accurately maintain client records and ensure they are logged correctly on the department's IT system.
5. Demonstrate strong report writing and communication skills.
You will also receive support through regular supervision and performance management sessions.
Qualifications
Along with a relevant qualification, you should have relevant knowledge and experience in Adult Social Care, either in a formal or informal capacity. You must be able to fully meet the requirements of the post and cover the whole geographic area of the City, either by possessing a current and valid driving licence or by access to public or other transport.
