An opportunity has arisen for a Clinical Pharmacist to join our pharmacy team within Wantage PrimaryCare Network.
Our Pharmacy Team, led by a Senior Clinical Pharmacist supported by two pharmacy technicians, take responsibility for areas of chronic disease management and undertake clinical medication reviews to proactively manage our patients with complex poly-pharmacy.
Our ideal candidate will be an experienced Clinical Pharmacist and ideally an Independent Prescriber however the opportunity to become a non-medical prescriber will be provided for the right candidate. You will work with the pharmacy team on quality improvement and clinical audits, as well as managing someaspects of the practices Quality and Outcomes Framework (QOF), Investment and Impact Fund (IIF) and Prescribing Quality Scheme (PQS).
Clinical supervision and mentorship will be providedto help support the development of the role .

The post holder will work within their clinical competencies as part of a multi-disciplinary team to provide expertise in clinical medicines management, provide structured medication reviews, including poly-pharmacy and people in residential care homes. They will manage long term conditions, management of medicines on transfer of care and systems for safer prescribing. They will manage repeat prescription authorisations, acute prescription requests, while addressing both the public health and social needs of patients in the Primary Care Network.
This role is pivotal to improving the quality of care and operational efficiencies so requires motivation and passion to deliver and excellent service within primary care.
About us
We are a two practice Primary Care Network with a combined population of circa 34k. We share the same premises at Wantage Health Centre which has recently undergone an extensive development to provide additional consulting rooms and modernisation of the reception and administration areas. There is ample free parking on site but also good access to public transport. Wantage is a growing market town at the foot of the Ridgeway mid Oxford, Reading and Swindon.
Both practices are teaching practices with a commitment to mentoring and supporting their clinical staff. We would provide all appropriate clinical supervision and would be supportive in encouraging the successful candidate to undertake further training and development.
We have an ongoing staff welfare programme and encourage all members of the team to come together for morning briefings, coffee mid morning and lunch time walks.

Key Duties and Responsibilities
Long-term conditions consult with those patients with single or multiple problems where medicines optimisation is required. Review the on-going need for each medicine and support patients ensuring they get the best use of their medicines. Make appropriate recommendations to the Senior Pharmacist or GPs for medicine improvement.
Medication Reviews Perform medication reviews of patients with poly-pharmacy, especially for older people, including residents in care homes and those with multiple co-morbidities and produce recommendations for the primary care team on prescribing and monitoring. Provide and support care home staff to improve safety of medicine ordering and administration.
Hospital Admissions planned and unplanned - Support the practice teams with hospital discharge prescribing arrangements, providing specific advice for those patients on multiple medications.
Service Development & Quality Improvement - Contribute pharmaceutical advice for the development and implementation of new services that have medicinal components advice on treatment pathways and patient information leaflets.
1. Undertake clinical audits of prescribing as directed by the GPs, feedback the results and implement changes in conjunction with the practice teams.
2. Implement changes to medicines that result from MHRA alerts, product withdrawal and other local and national guidance.
3. Monitor practice prescribing against local Prescribing Guidelines and Shared Care Protocols. Audit NICE compliance.
4. Provide education and training to the primary healthcare team on therapeutics and medicines optimisation.
5. Work with the practice teams to ensure compliance with CQC standards where medicines are involved.
Collaborative Working Arrangements - Participate in PCN MDT meetings. Liaise with ICS / ICB colleagues on prescribing related matters to ensure consistency of patient care and benefit. Foster and maintain strong links with all services across the PCN and neighbouring networks. Liaise with other stakeholders as needed for the collective benefit of patients.
Miscellaneous - Work as part of the team to seek feedback, continually improve the service and contribute to business planning. Undertake any task consistent with the level of the post and scope of the role, ensuring that work is delivered in a timely and effective manner. Duties may vary from time to time without changing the general character of the post or the level of responsibility.

Essential
6. Master's Degree in Pharmacy - MPharm or equivalent
7. Specialist knowledge through a Postgraduate Diploma and
8. qualifications, clinical, community, therapeutics or equivalent
9. Registered with GPhC
Desirable
10. Clinical Diploma
11. Independent prescriber status or a commitment to undertake the course
12. Knowledge of IT systems, including MS Office and use of EMIS, Docman
