· To provide high quality emergency care and minor injury treatment to patients with both primary care and acute care needs in a prison setting by playing an active part in the assessment, planning, implementation and evaluation of the patients care.
· This role requires a Registered Paramedic to work as part of a team delivering care to patients both within the Healthcare Centres and on the House blocks within the prison environment.
· The Registered Paramedic will work as part of a team of Primary Care Nurses within a HMP/YOI site.
· The role requires you to function autonomously and within your sphere of competency, whilst being a support and resource to less experienced members of staff. You must be able to assess patients, plan their care, administer treatment and discharge them or refer them as appropriate to other healthcare professionals / external agencies. The HMP/YOI sites are clinical facilities which require an individual to have the ability to work effectively as part of a multi professional team within a fast pace environment, in line with the protocols and policies of the local Service and CNWL, and professional bodies.

Clinical Professional Responsibilities:
• Act as a positive role model to staff and patients.
• Conduct invasive procedures where appropriately trained and competent
• To respond to all emergency codes in accordance with agreed protocols. This will include being the emergency responder.
• Procedures where appropriately trained and competent i.e. suturing, IV access, haemorrhage control, wound care.
• Vital observations and diagnostics eg blood pressure, pulse, SATs, blood glucose monitoring.
• Cardiac & CVA intervention / competent in the monitoring of physiological observations by undertaking a full assessment using a structured and prioritised approach and the recognition of the acutely ill patient e.g. ABCDE approach, ECG recognition, pathways for CVA and the diagnosis and referral of a range of cardiac conditions
• To determine the appropriate destination for the patient to include the initiation of treatment linked to an agreed and regulated care pathway in association with other health, mental and social care professionals in cases where transport to hospital is not required.
• Assess individual’s health needs.
• To undertake first and secondary reception screens.
• Provision of holistic care.
• Administer treatment within local guidelines / protocols within one’s own scope of practice.
• To maintain competencies associated with the role ensuring you are working within your own limitations / scope of practice.
• Administer medication, within areas of competence (as prescribed on SystmOne and homely remedies).
• Assisting with health promotion, screening and treatment clinics. Provide patient information and education.
• Develop / maintain advanced clinical skills.
• Referral / sign posting to appropriate health professionals / services.
• Assist with the activities of daily living and development of care plans for prisoners with physical and mental health needs.
• Ensure CNWL clinical protocols are adhered to at all times.
• Work at all times within the relevant code of professional conduct.
• Maintain own personal and professional development plan, in line with CNWL’s Trust and service objectives.
• Appraise own performance and discuss development needs of the individual and the team with mentors, supervisors and line manager. Appraise more junior staff members as applicable to the role / structure.
• Participate in regular clinical supervision.
• Be conversant and comply with the service’s fire precautions and procedures for evacuation and emergency resuscitation.
• Assist with the triage role as appropriate
• To maintain standards of infection control within the unit and during clinical care and to assist patients to maintain their own infection control needs.
Managerial:
• Prioritise patient care throughout the units, assuming shift leader responsibility allocating staff and resources effectively, as and when rostered to this function.
• Assist in orientation and support to new staff to the units.
• Support junior staff with supervision and development.
Communication:
• Ensure that identified lines of communication are maintained with patients, colleagues and external agencies to demonstrate politeness and courtesy and sensitivity promoting the corporate image of Central and North West London NHS Foundation Trust.
• Communicate and liaise with all healthcare staff.
• Play an integral role in ensuring that junior team members are aware of local and general Health & Safety matters, Clinical Governance and other on-going issues, which arise including patient safety issues.
• Provide and organise support through the means of clinical supervision in accordance with Central and North West London NHS Foundation Trust company policy.
• Demonstrate the ability to be able to motivate and persuade others through advanced communication skills.
• Effectively communicate with line managers.
• Clinical Governance
• To participate and assist in the systematic monitoring, review and evaluation of paramedic practice through clinical audit.
• To participate and assist in the setting of standards of patient care based on sound research findings.
• Work within CNWL’s Clinical Governance framework.
• Take a personal responsibility to maintain HCPC registration and professional development in line with the expectations of the role.
• Promote clinical excellence and best evidence based practice reflecting CNWL’s nursing philosophy and HCPC Guidelines.

• To research, develop, introduce and apply up to date research-based findings to paramedic practice, thus ensuring best evidence based practice is followed.
• Show commitment to lifelong learning and a learning environment through personal example and dissemination of skills, attitude and knowledge to others.
• Maintain registration with the appropriate professional body (HCPC) and professional development in line with the expectations of the role.
• Comply with the Care CNWL appraisal system and attend appraisal annually.
• To participate in and deliver training for junior staff..
• Ensure updated mandatory training requirements are met and equipment training records are maintained.
Quality Issues:
• Promote clinical excellence and best evidence practice reflecting CNWL’s clinical philosophy.
• Participate in annual review of protocols and evidence based documentation.
• Participate in Primary Care clinical audit strategy.

Responsibilities Specific to the Prison Environment:
• Comply with all relevant security procedures pertaining to keys and the passing on of relevant information which might pose a risk to staff, other patients or third parties.
• Be familiar with Prison Service Standards, Standing Orders, Circular Instructions, and Notices to Staff, Prison Rules, Health & Safety and any other relevant material in order to function as an informed practitioner with the prison setting.
Working Conditions:
• Staff may be exposed to distressing or emotional circumstances such as information regarding patient’s criminal behaviour or occasionally exposed to distressing episodes of self-harm.
• Staff will be required to concentrate for periods of time where the work is unpredictable.
