Opportunities have arisen for new appointments within the Public Health Nursing Team within Livewell Southwest.
You will be entering the Specialist Community Public Health Nursing Service at a time of exciting changes and opportunities. All Livewell staff are expected to be able and willing to work across a 7-day service.
Main Duties of the Job
1. Responsible for ensuring the co-ordination and delivery of skilled, effective care to promote positive health and respond to identified individual client and population/community needs.
2. Undertake an ongoing community health needs profile of the caseload identifying individual, family, and community needs affecting health and social well-being.
3. Work with vulnerable children, young people, and families in line with national and local policies relating to safeguarding children.
4. Work in partnership with other agencies, particularly health services, education, and social care.

Person Specification
Knowledge
* Demonstrate knowledge of current health issues relating to children and families.
* Knowledge of Children Act, Healthy Child Programme (0-19), and Child Protection policies.
Experience
* Experience of working with children and families.
Skills
* Ability to work effectively with a multi-disciplinary team.
* Self-manage time and be adaptable.
Qualifications
* Specialist Community Public Health Nurse Certificate, Diploma, or Degree.
* NMC Registration.
