This is a uniqueand rewarding role which will allow you to work with, lead and supportcolleagues during a time of innovation and development as our team focusses onthe implementation of Urgent Community response and of Virtual Wards.
You will workwithin Directorate One of Community Services, supported by senior colleagueswho are passionate and proud of the services we deliver to our community ofSouth Tyneside and Sunderland.
The team deliversplanned and unplanned care to people in their own homes, to assist inpreventing hospital admission and supporting those who have recently beendischarged from hospital, require enhanced intervention and short-termrehabilitation support for their physical and functional health needs to remainat home.
