This role offers a unique opportunity to work with a team dedicated to transforming services for individuals with learning disabilities, promoting independence, and improving quality of life.
You will play a key role in ensuring that people with learning disabilities receive personalized care and support, enabling them to achieve their potential.
Key Responsibilities:
1. Carry out assessments to identify the needs of individuals with learning disabilities and create tailored support plans.
2. Work collaboratively with a multidisciplinary team, including health professionals, housing services, and families, to ensure a holistic approach to care.
3. Advocate for individuals with learning disabilities, ensuring their voice is heard and their rights are respected.
4. Support the development and implementation of innovative, person-centered services that enhance the lives of people with learning disabilities.
5. Monitor and review support plans regularly, adapting them as necessary to meet changing needs.
6. Stay informed on best practices and legislative changes, incorporating this knowledge into everyday practice to ensure high-quality care delivery.
Requirements:
7. Qualified Social Worker with valid Social Work England registration.
8. Proven experience working with individuals with learning disabilities.
9. Strong understanding of the Care Act, Mental Capacity Act, and other relevant legislation.
10. Ability to work both independently and as part of a team, with a proactive approach to problem-solving.
