Llantrisant Dialysis Unit is a dynamic 21 station unit providing haemodialysis treatment for 84 patients inpartnership with the NHS Trust. The unit operates safe staffing with aprotected 3:1 patient to nurse ratio.
You will be fully supported andprovided with excellent training and development which will enable you toprovide safe, high quality, holistic nursing care to patients undergoingdialysis treatment. Main duties of the job
To develop and expand theoretical knowledge and practical skills involved in the care of a dialysis patient
To demonstrate competent and efficient patient assessment care needs, coordinating with the unit link role resources and multidisciplinary team, to ensure patients receive their optimum care
To demonstrate a proactive mentorship approach to the team, acting as a resource in the unit team, empowering patients, qualified nurses, ancillary staff and student nurses to share knowledge and skills.
Hours of work:
Full time role working hours per week
3 x hour shifts per week plus an additional top up shift every month
Long Day = 06:30 -18:00
Monday to Saturday on a rota'd shift system
No Sunday working
No night shifts
We expect staff to work flexibly and work shifts depending on the need of the business

Job responsibilities

- to complete the Avitum inductionprogramme within 3 days of employment start date.
- to undertake the Avitum staffdevelopment phased programme. This will be successfully completed within anagreed time frame with the Dialysis Unit Manager.
- to complete the Avitum annualmandatory training requirements.
- to attend Avitum training and studydays as scheduled.
- to achieve annual performancemanagement objectives and gateway transitions.
- optional enrolment in the access,renal & management courses.
To demonstrate an active andeffective clinical practice, providing a safe, high quality care for patients,achieving compliance with established procedures and codes of conduct andpractice at all times, recognising and accepting individual skill and knowledgelimitations and reporting when medical intervention is necessary: -
- to demonstrate commitment toself-development, updating staff development training file.
- to provide evidence of postregistration education practice as per Nursing & Midwifery Councilrequirements to maintain an active registration.
- to liaise with Dialysis UnitManager, unit trainer, mentor and Training & Development Manager toestablish appropriate annual performance management objectives.

- to demonstrate a competent andaccurate review of the patients coagulation management, with activated clottingtime monitoring, providing evidence of evaluation care to optimise clinicalstandard achievement.
- to demonstrate a competent andaccurate review of the patients anaemia management, providing evidence ofevaluation care to optimise clinical standard achievement.

To demonstrate accurate and competentevidence of the holistic dialysis care provided as a named nurse. To maintainan in-depth knowledge of the patients physical, psychological and social state,promoting the patients interests as a nurse advocate :-
- to maintain an awareness of alldevelopments in the patients clinical timeline outpatient appointments,referrals, investigations, multidisciplinary team etc.
- to interact with the patient in theplanning of their care to optimise empowerment and independence.
- to perform documented holisticassessments of the patient at least every 3 months.
- to identify problems and potentialproblems in conjunction with the patient, providing support in developing plansto address any issues.
- to competently select theappropriate care pathway for identified problems.
- to monitor access management 3monthly and review results.
- to monitor activated clotting timeranges 3 monthly and review anticoagulation.
- to seek to interact with thepatients family in their care, as per patient consent.
