North East Derbyshire Primary Care Network Ltd (NED PCN) has an exciting opportunity to join our PCN team. At NED PCN we already employ Home Visiting Clinicians who work to care for patients across our four member practices. Existing team members have backgrounds as Community Matróns and Paramedics. The Clinicians play an integral role within our general practice teams, which is why we are looking to expand our PCN workforce.
Main duties of the job
The role will involve developing continuity relationships with our housebound and care home patients, taking a holistic approach to their management. It will involve true MDT working, building relationships with GP practices, Care Homes, Community Rapid Response services and Falls services, to help clinically deliver the service on a day-to-day basis.
This is an evolving service, so you will have the opportunity to contribute to the development of the Home Visiting Service if you wish to.
The postholder will act autonomously within their professional scope of practice providing care for patients face to face, telephone consultations and home visits.
The role includes the provision of weekly planned care home ward rounds.
The postholder will operate within their own sphere of competence and agreed acceptable limits of practice to provide expert professional clinical advice to patients, carers and colleagues, ensuring clinical safety and excellence.
The practitioner will need to prioritise the needs of patients, making any necessary referrals for investigations in the appropriate manner.
The practitioner will be supported in their role by Community GPs and also GPs within the practices within the PCN. There will be opportunities for upskilling and further training within the role to meet the needs of the service if necessary. Job share and part-time applications are also welcome.
About us
NED PCN is an organisational group of four proactive GP practices in North East Derbyshire. The Practices are Barlborough Medical Practice, Killamarsh Medical Practice, The Springs Medical Partnership and The Valleys Medical Partnership.
NED PCN is a relatively small PCN with just over 40,000 patients. We feel that this makes us big enough to develop a successful team but small enough to be an approachable, friendly place to be.
All our practices are close to the M1 and commutable from major cities including Chesterfield, Sheffield, Derby and Nottingham.
The NHS Long Term Plan describes the prominent role Primary Care Networks play in delivering proactive, personalised and more integrated health and social care for their local populations. This will require collaborative working between organisations including GP practices, acute and community health, social care organisations and the voluntary and community sector.
