The Children’s Transitions Team is based in Ellesmere Port and is co-located with wider children’s social care, early help and education teams. The Children’s Transitions Team works with young people aged 16-19 years with disabilities. We work across the whole borough of Cheshire West and Chester so there will be a requirement to travel. The family intervention worker role is wide and varied and involves working directly with young people and their families within their own homes and in the community, as we support them to transition from children’s social care to adult’s social care. This role will entail working from both a children’s and adult’s social care perspective. The Family Intervention Worker will be involved in supporting our young people and their families to access appropriate services, promote independence, and understand the services eligible to them within this age range and as they transition into adult’s social care. They will complete assessments of need, risk and strengths within the family to enable social care teams to promote services/support relevant to the needs of our young people and their families.
