This is an exciting opportunity to be part of a team putting people first to make sure they get the right support at the right time, ensuring the same day discharge for those individuals who are clinically ready to return home. As part of our Home Assessment team, you will work alongside Health & Social Care and Acute colleagues whose focus is prevention of delays in hospital discharge arrangements.
Working alongside the individual and their families and other support networks ensuring good quality support in those first days after returning home. Within the North Lanarkshire Partnership our ambition is beyond same day hospital discharge, but prevention of admission at the front door of the hospitals and within community, where response to crisis situations can prevent hospital admission. As the Home Support Team Leader, you will be at the forefront of this new initiative.
Your role will include the operational responsibility for ensuring our community-based home support workers across North Lanarkshire deliver a high standard of responsive and proactive care to service users and their families. You will assist the Service Delivery Manager implement and maintain effective systems of accountability, quality assurance and performance management, ensuring all personal support plans are monitored and reviewed within statutory timescales. As this role involves co-ordinating and managing a team of dispersed Home Support Workers it is essential that you can demonstrate that you have excellent organisation, management and leadership skills to ensure effective use and management of resources.
Extensive working experience within a social work, home support, voluntary or independent sector setting is essential. Possession of a SVQ Level 3 in Health & Social Care or equivalent is desirable, successful applicants who do not currently hold a relevant qualification will be required to complete this within 3 years of commencing in post.
You are required to become a member of the Protecting Vulnerable Groups (PVG) Scheme prior to commencing in this role and be registered or eligible for registration with the Scottish Social Services Council (SSSC).
Work pattern: 17.5 hours.
Week 1 – Wednesday, Thursday, Friday
Week 2 – Monday, Tuesday
