Your Role
We have an exciting opportunity for three ARCH/Discharge Social Workers to join our supportive team. In this role, you will work independently to facilitate patients' transitions back into the community, emphasizing the "Home First" ethos.
The position involves discharge planning and multidisciplinary team (MDT) working from the hospital wards. You will also support the Transfer of Care hub when required and have the opportunity to support across our Discharge to Assess (D2A) bed-based provision. Additionally, we have a Reablement community team who work alongside our provider Service offering Care Act assessments and support planning within the Community.

About You
We are looking for an experienced Social Worker in Adult Social Care. You will take a people-first approach and utilise strength-based practice. You should be able to work effectively both as part of a team, including within a wider multi-disciplinary setting, as well as independently. The role requires flexibility to meet the demands of hospital discharge while providing appropriate support and information to help individuals transition out of the hospital in line with discharge guidelines.
