Social Worker in Recovery and Rehabilitation Service

Main duties of the job
Therole will involve working as part of a wider multi-disciplinary team (MDT)supporting people through the Rehab pathway in both inpatient communitysettings upon discharge, using your expert Social Work skills and knowledge in assessment,support planning and commissioning in line with the expectations of the CareAct 2014 and S117 aftercare planning.

* The post holder will deliver high qualitycomprehensive mental health services. The principal purpose of the job is to improve thepsychological health, wellbeing and social functioning ofpeople with complex psychosis within an identified Devon locality.
* The post holder will join a place basedRecovery and Rehabilitation Service delivering recovery focused, evidence leadinterventions and support for individuals with complex psychosis, theirfamilies and carers. The post holderwill operate flexibly across the domains of the service (intensive inpatient,intensive community deliver, ongoing community delivery and wider place baseddevelopment).
* Thepost holder will work as part of a specialist rehabilitation service working with a clinical caseload of individuals with complex psychosis,using specialist,social work and social care focussed evidence based assessment in line with theprinciples of the Care Act 2014.
* The post holderwill act as a lead specialist for the Recovery and Rehabilitation service,providing professional clinical supervision to less experienced staff, trainees& students, and contributing to the leadership of the local service.
* The role involves managing a defined caseloadof individuals with a focus on providing expert assessment and treatment forpeople with a complex mental health diagnose. This will include recoveryco-ordination (design and co-ordination of treatment packages that arepersonalised). The post holder will provide statutory Adult Social Care, CareManagement functions (assessment, care planning and reviewing) and liaison witha wide range of statutory and non-statutory agencies is central to this role.
* The post holder will contribute to the clinicalleadership of the team, providing supervision for other members andparticipating in team and service development, and evaluation.
* As an inpatient and community RehabilitationSocial Worker you will assess and plan support for a range of complex casework, as well as providing professional advice, mentoring and support to yourcolleagues, newly qualified social workers and student social workers. We havean expectation that you will have a good working knowledge of social carelegislation (The Care Act 2014 and its interface with S117 MHA 1983/2007).
* As part of the MDT you will undertakesafeguarding enquiries where adults are at risk of abuse or neglect and withsound assessment skills, undertake Mental Capacity Assessments that inform BestInterest decisions to ensure that people are living as independently aspossible.

Person Specification
Qualifications
* A professional mental health qualification eg Social Work Degree or equivalent, RMN or BSc/Diploma in OT, SROT, Chartered Clinical Psychologist, on the appropriate professional body register and hold current registration with no restrictions.
* Evidence of relevant continuing professional development and training related to the role
* Training to level 3 in Safeguarding children and young people: roles and competencies for Health Care staff
* Experience and ability to demonstrate working as a registered social worker in line with the Social Worker level in the PCF
* Post graduate qualification relevant to specialism
* Evidence of leadership/management training within a core service
* Mentoring students in line with professional requirements
Experience
* Post registration experience (of two years) working in either a social care or mental health placed setting with inpatient work an advantage.
* Substantial experience of managing/leading a busy caseload of clients with a range of health and social care needs within a community setting and without supervision;
* Working within a community mental health or social care setting in either the statutory or voluntary sector.
* Experience of clinically supervising junior and unqualified staff and mentoring students
* Experience of working in a multi-disciplinary and multi-agency environment.
* Participation in user involvement in services.
* Experience in deputising for more senior staff
* Evidence of following through specific pieces of work or projects as delegated by ward managers or others
* Experience of working with individuals who are misusing substances including alcohol
Knowledge
* An understanding of the NSF and Mental Health Act/ Mental Health Capacity Act requirements.
* An understanding of social care legislation (Care Act 2014)
* Awareness of community structures and social care management responsibilities
* Understanding of recovery principles in Mental Health
* Knowledge of treatment of drug /alcohol dependence and impact of substance misuse on a persons mental health
skills and abilities
