To play a proactive role on each admission, introducing themselves and their role to all patients and carers. To facilitate in the daily bed allocation meeting and Hospitals of Birmingham and Solihull (HOBS) palliative care meeting calls. Form part of the clinical administration team and deputise for the clinical admin team leader. Support ward staff to ensure that discharge planning is proactive, timely and efficient. Liaise effectively with statutory and voluntary agencies in relation to the patients discharge arrangements. Monitor length of stay and escalate any delays in discharges. Ensure appropriate external referrals have been made e.g. Social Services, Continuing Healthcare, Care Agencies. Communicate with patients, their families and the person who referred this patient. To verify all parties, understand admission plans, dealing with their concerns in a compassionate and effective manner. Ensure that relevant forms for Social Services or Continuing Health Care funding (CHCC) are completed by the multi-disciplinary team. Ensure the discharge documentation is kept up to date. Work closely with all of the multi-disciplinary to ensure admission and discharge plans are in place and communicated to all of the team. Engage and continue conversations with families linked to discharge. Coordinate with pharmacy and medics to ensure that medications required on discharge (TTO) are ready for day of discharge. Liaise with therapy team to ensure any necessary equipment is in situ. Ensure all discharge paperwork is completed for day of discharge and copies are sent to GP, District Nurse, Ward Clerk and Patient (this is not an exhaustive list). To complete the follow up call to the patient the morning following discharge. To complete Condolence Call to the Bereaved and make an initial assessment within a few days of the death about the needs for additional follow-up. Ensure that carer/family is aware of bereavement help points, support groups and counselling services, Send the Bereaved family a condolence card, and any other information identified from the follow up call to support them. Develop and maintain effective communication and positive working relationships with the nursing and multi-professional teams and with other services of St Giles Hospice. To contribute to caring and safe atmosphere for patients and carers. If required, work in partnership with patients and carers, under the supervision of a registered nurse, giving essential nursing care within the agreed competencies and boundaries of the role (HCA, NVQ level 2 or 3). Be able to complete a proactive risk assessment e.g. Health and safety issues relating to the IPU environment. To maintain accurate, contemporaneous and complete records in accordance with Hospice policy Attend multi-disciplinary meetings when required playing a leading role in ensuring that discharges are effectively communicated on and planned, completing all records and facilitating the completion of any relevant actions from such meetings.
