We are looking for a Care Liaison advisor to provide administrative support functions to the scheme by working directly with families. Sourcing both short term and long-term care home placements/packages of care for clients working towards a quick, safe, and correct discharge plan for patients.
This position offers a unique opportunity to work towards quick and safe discharge plans, supporting families during challenging times.

Every day you will…
* Liaise with care homes, care agencies, senior health and social care staff, patients, and their families.
* Provide and communicate updates and act as the link between discharge teams, local authority, clients and care providers to ensure the successful delivery of a high-quality service whilst ensuring discharge happens within set timescale.

To thrive in this role you must ideally have...
* Previous customer service experience with excellent written and verbal communication skills.
* Previous administration experience with good attention to detail.
* The ability to organise and prioritise workload effectively.
