We currently have an opportunity for an aspiring or established Community Mental Health Nurse based in Woking Community Mental Health Recovery Service (CMHRS).
We are a multi-disciplinary Recovery based community mental health service and provide evidence based treatment and interventions to all people who use our service with a focus on Recovery and promoting independence. We are working towards providing treatment within structured Care Pathways that ensure the best outcomes for our clients. Team is fully engaged in an exciting Community Mental Health Transformation Programme are looking to provide innovative approaches to meeting the mental health needs of a diverse community.
You will be a registered Mental Health Nurse, required to demonstrate a high level of commitment, motivation and knowledge of working with people with severe and enduring mental health problems. You will be supported in your role not only through formal supervision and training but also through a strong multidisciplinary and collaborative approach within our team.
Main duties of the job
This post will include assessing new people referred to our service and working with an allocated caseload of existing service users providing low level psychological interventions and liaising with other professionals to coordinate their care under the Care Programme Approach. You will need to be able to safely administer medication in a community setting, engage in medicines management and physical health monitoring.
You will need to demonstrate excellent communication skills and be able to provide clear information around health promotion and psychoeducation to people who use our services as well as providing educational input and supervision to nursing students and junior colleagues.
You will need to be able to work flexibly and collaboratively with a multidisciplinary team and other agencies. You will need to be able to assess risk dynamically and work to safeguard our clients at all times. You will work in our Team Base and in people's homes.
The candidate must have a full and valid driving licence and access to a vehicle for work purposes due to the community element of this role.

1. 5 years experience working with adults with Mental Health Problems (including training), or additional relevant post registration training.
2. Completed a relevant post registration preceptorship programme.

1. Experience working in either in primary or secondary community mental health care.
2. Able to carry out supervision.
3. Able to co-facilitate group work.
4. Able to use IT confidently.
