You will support our existing team of Clinical Nurse Specialists based in the hospice and will work closely with health care teams and hospital specialist palliative teams. You will provide specialist expertise and support to patients with advanced and progressive life-limiting illnesses and their families.

Main duties of the job
* Work with the Community Clinical Nurse Specialist team.
* Work closely with our clinical external agencies.
* Provide clinical direction and support to the CRNs, Day Hospice, and Wellbeing.
* Be the initial line of support for patients and families, providing advice, signposting services, considering advance care planning, and providing support in times of distress.
* Provide the point of access for other health and social care professionals providing relevant end-of-life support and advice as well as signposting education opportunities.

Person Specification
Qualifications
* NMC part 1 or 2 and recent experience of nursing patients with palliative care needs or oncology would be essential as is experience of caring for families and patients facing the challenge of terminal illness.
