This is a fixed term post or secondment opportunity to support the NHS England funded Managing Heart Failure @ Home pilot project.
Main duties of the job
The position involves home visiting to assist Heart Failure specialist nurses in delivering holistic and comprehensive care.
Duties include conducting ECGs, attaching ambulatory diagnostic devices, and recording basic measurements like height, weight, and blood pressure. Additionally, the role entails drawing blood samples, both venous and capillary, for Point of Care Testing (POCT), aiding the nurses in managing heart failure symptoms and minimizing hospital admissions.
Exceptional communication abilities, teamwork, direction-following, initiative, and organisational skills are crucial. Collaboration is a significant aspect of this job, necessitating effective interaction within a diverse, multidisciplinary team.

Qualifications and professional training
Essential
1. Nursing Associate Foundation Degree or equivalent training/experience
2. GCSE Grade A-C Maths and English or skills level 2 Maths and English or equivalent.
3. Phlebotomy and venepuncture competencies / training

4. Registered Nursing Associate on NMC register
Experience and knowledge
Essential
5. Understanding of the scope of the role of the Nursing Associate in context of the nursing and interdisciplinary team and the organisation, and how the role may contribute to service development
6. Understands and acts in line with NMC professional standards for practice contained within Standards of Proficiency for Nursing Associates.
7. Have a level of knowledge of heart failure inclusive of causes and treatment along with basic knowledge of a broad range of conditions experienced by patients in community and general practice settings. This will include Long Term Conditions, and common mental illnesses, frailty and palliative and end of life care.
8. An understanding of the presentations of multiple pathology, depression and anxiety states and frailty, predominantly in older people.
9. Knowledge of when to seek advice and escalate to the appropriate professional for expert help and advice
10. Understand the importance of following procedures and treatment plans
11. Experience of working in teams under appropriate supervision as part of a multi-disciplinary team
12. Insight into how to evaluate own strengths and development needs, seeking advice where appropriate
13. Experience of providing and receiving complex, sensitive information
