Join to apply for the Nurse Practitioner for Older People role at Kumu Wellness.
We are recruiting to a team of dedicated community nursing staff to work with ICCT Service and our Primary Care Networks. This exciting opportunity means that you'll be working as part of a combined team of community care staff and PCN colleagues to proactively support people with moderate and severe frailty as part of WCHCs Population Health Management.
You will be based within the ICCT service working 22.5 hours per week. This is a permanent position.
This post requires good communication and interpersonal skills and the ability to be adaptable, reliable and approachable. You will be working in both lone capacity and in conjunction with other team members, ensuring clinical standards are maintained using evidence-based practice. You will have a good knowledge of the components of Clinical Governance, Best Practice, Risk Management, Clinical Audit and relevant Professional Codes. You will be able to evidence excellent clinical skills.
We believe that investing time with people with more complex needs will not only improve their quality of life, but will also reduce the pressure on unplanned care. The team will continuously review activity and outcomes to show the difference care provision is making to patients and the wider health and care system. This will inform how Wirral plans its future proactive care services.
As part of this team, you will be actively working to ensure we recognise those people who will benefit from person-centred and holistic assessment and care planning, and then receive expert, compassionate, well-coordinated support. (This is often described as the process of Comprehensive Geriatric Assessment, or CGA.)
Please review the person specification and ensure you detail how you meet each point.
