Job Title: Care Coordinator
Location: East Fife
Salary: £23,100pa
Hours: Full time hours, Monday to Friday - 9am to 5pm with flexibility (plus on-call expectations)
This role will require the successful candidate to be able to drive and to cover care calls if required.

Your role at Prestige:
We are looking for a dynamic Care Coordinator with solid planning and rostering experience that is matched to a flexible, enthusiastic and can-do attitude to join our East Fife team.
You will be assisting and supporting your Branch Manager in ensuring that key business and compliance targets are met.

The role of Care Coordinator is always to promote Prestige Nursing & Care as well as ensuring that the utilisation of nursing and care resources is both maximised and aligned with client requirements within defined quality, compliance and safety standards.
The success of this role will be measured by:
1. Member utilisation targets are achieved
2. Effective shift rostering including forward planning resulting in all clients having suitably allocated carers
3. Identifying business growth opportunities
4. Forming strong relationships with the Carers
5. Business KPI's met
Responsibilities:
1. Ensuring that all client visits are covered on a daily basis.
2. Ability to liaise with a variety of healthcare professionals, social workers, and case managers.
3. Ensure compliance with all office systems and procedures.
4. Take ownership of area and work in collaboration with the other care planners to ensure care is covered.
5. Communicate with our frontline colleagues regarding changes relating to client's needs.
6. Maintaining great relationships with existing clients and colleagues
7. Identify new business opportunities to support growth with the Branch Manager
8. Work in cooperation with members of multi-disciplinary health teams so as to maximise opportunities for clients care needs.
9. Ensure the effective placement of frontline colleagues to meet client needs
10. Prepare ad hoc reports as and when required
11. Undertake the on-call rota as per the needs of the business
The role is branch based but may require some travel to other branches/sites to meet the needs of the business.
General Responsibilities:
