Lead Nurse - Adult Community Mental Health, Derby City Well Being Team
It is an exciting time to work in the Derby City Wellbeing Service for Adults of Working Age, as we are in a period of transformation in line with the Community Mental Health Framework (Living Well).
This post will be working primarily within the Long Term Offer, offering CORE assessments and care co-ordination to people with SMI and people with complex mental health needs.
We are based just a short walk from Derby City Centre with opportunities to work from home some of the time.
We will offer you robust clinical and managerial supervision, as well as ongoing training, whilst supporting you to develop within your role/interests, with support from our clinical lead, peers and manager.
We seek individuals who are warm, engaging and focussed on person-centred care.
You must hold a Mental Health nursing qualification and have experience post qualification and will be a good communicator, demonstrating respect and dignity.
We focus on empowerment and recovery for the service user and their families.

We would welcome and recommend informal visits or a discussion with the appointing manager with prior arrangement to meet our team prior to application/interview.
Main duties of the job
The post will entail working with people who are experiencing severe and enduring mental ill health and enabling them through their recovery journey.
Ideally, you will have experience of community work and working with people with enduring and complex mental health needs. You will work with a caseload of service users and support the duty function within the team. At times, you will also be expected to work with the short term offer within the Derby Living Well Service, offering short term assessments (initial contacts) and plans of care.
You will need thorough knowledge of the Care Programme Approach and be able to demonstrate expertise in risk assessment, care planning and management working in a community setting.
It is essential that you have good communication skills and that you operate as a well-integrated member of our friendly, multi-disciplinary team, maintaining a person-centred focus.
As a lead nurse, you will have demonstrable RMN post registration experience and be able to demonstrate post registration study and development.
You will also need to demonstrate expertise around NMC nurse competencies.
You will be expected to be a role model, offering mentoring and supervision to other staff and students.
You will need to have the ability to transport yourself and service users across a wide geographical area without compromising service needs.
