Job responsibilities This list is not exhaustive and will flex and change to meet the demands of the practice, and with the successful candidate depending on what skills they currently hold: The role of the diabetes nurse is to promote the health and wellbeing of people with diabetes, to encourage compliance with medication through informed self-management. The post holder with work with and liaise on a regular basis with the Practice GP diabetic leads and nursing team to manage patients with diabetes and to assist in the development of the diabetes service. Identify clinical risks and take appropriate action. Provide a pathway of diabetes nursing care for the patient, referring to secondary care, multidisciplinary teams and voluntary services as required.
