About the Role As a Support Practitioner, your responsibilities may include: Social Support - Helping individuals engage in their communities through outings to day centres, cinema, swimming, and other activities. Personal Care - Assisting with bathing, dressing, dental/doctor visits, and incontinence care to promote wellbeing. Practical Support - Supporting daily household tasks such as cleaning, laundry, ironing, cooking, and shopping to help maintain tenancies.
