An exciting opportunity hasrisen to work as an Advanced Nurse Practitioner within a committed team workingacross two sites in Chislehurst. You will work collaboratively within thePractice team to meet the needs of patients, supporting the delivery of policyand procedures, and providing nurse leadership as required. In order to work atthis level NMC requirements for advanced practice must be met.
Main duties of the job
Weare looking to employ a part-time experienced Advanced Nurse Practitioner tosupport our large patient list. You should feel comfortable workingindependently as well as alongside our multi-disciplinary medical team. Alwaysacting within your professional boundaries, You will provide care for thepresenting patient from initial history taking, clinical assessment, diagnosis,treatment and evaluation of care. You will demonstrate safe, clinicaldecision-making and expert care, including assessment and diagnostic skills,for patients within the general practice. The ideal candidate will demonstratecritical thinking in the clinical decision-making process.
Youwill possess the necessary skills, knowledge and behaviour by promoting andprotecting the interests of service users and carers, communicatingappropriately and effectively, working within the limits of your knowledge andskills, refer & delegate appropriately, respect confidentiality, managerisk, report concerns about safety, be honest and trustworthy and keep recordsof your work.
The role is for 30 hours per week, must be ableto work Monday and Friday, can be flexible on the other days. The successfulcandidates will be expected to work across the two sites.

The Chislehurst Partnership is a friendly high achieving suburban partnership in the London Borough of Bromley with a countryside feel. Currently operating from two sites (The Chislehurst Medical Practice & The Woodlands Practice). The Partnership is passionate about providing a friendly, supportive and caring medical service to our population. The Partnership hopes to move to a purpose-built medical centre in the future but for the time being are operating from two premises.
We have a patient list size of circa 25,000. We are a 9 Partner partnership with a team of Salaried GPs, Advanced Nurse Practitioners and a Nursing team. Our staff are friendly and hard-working and together we provide a high level of care to the local community. We are a well organised, progressive, forward-thinking Partnership. We are part of the Mottingham, Downham & Chislehurst (MDC) Primary Care Network and share across our practices a wide range of additional roles including, Clinical pharmacists, Physicians associates, Social Prescribers, a Mental Health Practitioner and Paramedic.

Job responsibilities
The ANP will have the following key job responsibilities, in delivering health services;
will work as part of a multi-disciplinary team within the practice.
, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis.
3. Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly
4. They will advise patients on general healthcare and promote self-management where appropriate, including signposting patients to the practices social prescribing service, and where appropriate, other community or voluntary services.
5. Provide clinical leadership within the practice
6.Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within scope of practice
7.Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
8. They will communicate at all levels across organisations ensuring that an effective, person-centred service is delivered.
9. They will provide an alternative model to urgent and same day GP home visit for the partnership.
10. They will communicate proactively and effectively with all colleagues across the multi-disciplinary team, attending and contributing to meetings as required.
11. They will maintain accurate health records appropriate to the consultation, ensuring accurate completion of all necessary documentation associated with patient health care and registration with the practice.

13. Communicate effectively with patients, and where appropriate family members and their carers, where applicable, complex and sensitive information regarding their physical health needs, results, findings, and treatment choices.
14.Diagnose and manage both acute and chronic conditions, integrating both drug- and non-drug-based treatment methods into a management plan.
15.Utilise and demonstrate sensitive communication styles, to ensure patients are fully informed and consent to treatment

20. Recognise and work within own competence and professional code of conduct as regulated by the NMC. Produce accurate, contemporaneous and complete records of patient consultation, consistent with legislation, policies and procedures
21. Deliver care according to NICE guidelines and evidence-based care
22. Assess effectiveness of care delivery through self and peer review, benchmarking and formal evaluation
23. Initiate and participate in the maintenance of quality governance systems and processes across the organisation and its activities
24. Utilise the audit cycle as a means of evaluating the quality of the work of self and the team, implementing improvements where required
25. In partnership with other clinical teams, collaborate on improving the quality of health care responding to local and national policies and initiatives as appropriate
26. Support and participate in shared learning across the practice and wider organisation
27. Use a structured framework to manage, review and identify learning from patient complaints, clinical incidents and near-miss events
28. Assess the impact of policy implementation on care delivery
29. Understand and apply legal issues that support the identification of vulnerable and abused children and adults, and be aware of statutory child/vulnerable patients health procedures and local guidance
